The straightening of the normal cervical lordosis, disc herniation at C4-5, diffuse spondylitic change with bilateral foraminal stenosis at C5-6, and disc herniation at C6-7 based on the MRI report. Next, multiple steps of the C5-6 anterior fusion surgery, including the right anterior neck incision, discectomy at C5-6, partial corpectomies at C5 & C6, insertion of fibular allograft, and placement of anterior cervical plate.
